The enteric nervous system (ENS), which consists of two subdivisions known as the submucosal nerve plexus and the myenteric nerve plexus, innervates and regulates functions of the gastrointestinal (GI) tract. Recent studies have focused on the involvement of the ENS in Parkinson's disease (PD), a disorder characterized by motor dysfunction.

A form of PD associated with late-onset symptoms is caused by a mutation in a gene encoding the protein α-synuclein that leads to the misfolding of α-synuclein proteins. These mutant proteins generally aggregate into clusters referred to as Lewy bodies, which accumulate in the central nervous system (CNS) over time, impairing motor neuron function (eg, reduced secretion of certain proteins and neurotransmitters) and eventually causing neuron death.

Scientists hypothesize that Lewy bodies in individuals with this mutation may originate in the ENS prior to the onset of PD symptoms. This hypothesis was tested in experiments using 2-month-old wild-type mice (WT mice) and 2-month-old mice with this α-synuclein mutation (MT mice) exhibiting no PD motor symptoms.

Experiment 1

For both WT and MT mice, muscle contractions and the speed at which food moved along the tract were measured in the small intestine (Figure 1).

Experiment 2

Equal numbers of cells (approximately 400) from the myenteric nerve plexus were isolated from WT mice and exposed to either a control solution lacking α-synuclein, 0.5 μM monomeric (nonaggregated) mutant α-synuclein proteins, or 0.5 μM aggregated mutant α-synuclein proteins. After 5 days, the total numbers of living cells were determined along with the numbers of these living cells expressing a protein called PGP9.5 (Figure 2), which is known to be expressed in healthy neurons.

The seal of the Knights Templar depicts two knights astride a single horse, a visual testament of the order's poverty at its inception in 1119. Nevertheless, these Knights of the Temple—who swore oaths not only of poverty but of chastity, loyalty, and bravery—would eventually become one of the wealthiest and most powerful organizations in the medieval world. So far-reaching was their strength and acclaim that their destruction must have seemed as sudden and surprising as it was utter and irrevocable. The signs of danger could not have been wholly invisible, however, as the Templars' growing influence became perceived as a threat to European rulers.

The first Templars were nine knights who took an oath to defend the Holy Land and any pilgrims who journeyed there after the First Crusade. Having secured a small benefice from Jerusalem's King Baldwin II, the knights inaugurated their mission at the site of the great Temple of Solomon. They quickly attracted widespread admiration as well as many recruits from crusaders and other knights. Within a year of its founding, the order received a financial endowment from the deeply impressed Count of Anjou, whose example was soon followed by other nobles and monarchs. As early as 1128, the Templars even gained official papal recognition, and their wealth, holdings, and numbers swelled both in the Holy Land and throughout Europe, especially in France and England.

However, this growing power contained the seeds of the order's downfall. Although the Templars were generally held in high esteem, the passage of time saw censure and suspicion directed toward them. The failure of the disastrous siege of Ascalon in 1153 was attributed by some to Templar greed. Similarly, in 1208 Pope Innocent III condemned the wickedness he believed to exist within their ranks. Moreover, their increasingly elevated status brought them into conflict with established authorities. One revealing example occurred in 1252 when, because of the Templars' "many liberties" as well as their "pride and haughtiness," England's King Henry III proposed to curb the order's strength by reclaiming some of its possessions. The Templars' response was unambiguous: "So long as thou dost exercise justice thou wilt reign; but if thou infringe it, thou wilt cease to be King!"

Ultimately, the impoverished Philip the Fair of France joined forces with Pope Clement V to engineer the Templars' downfall beginning in 1307. Conspiring to seize the order's wealth, Clement invited the Templar Master, Jacques de Molay, to meet with him on the pretext of organizing a new crusade to retake the Holy Land. Shortly thereafter, Philip's forces arrested de Molay and his knights on charges the preponderance of which were almost certainly fabricated. Ranging from the mundane to the unspeakably perverse, the accusations even included an incredible entry citing "every crime and abomination that can be committed." Suffering tortures nearly as horrific as the acts of which they were accused, many Templars confessed.

Not everyone believed these charges. Despite Philip's urging, Edward II of England remained convinced that the accusations were false, a view seemingly shared by other rulers. Nevertheless, Clement ordered Edward to extract confessions, a task the king tried to carry out with some measure of mercy. By 1313, the Templar Order had been dissolved by papal decree, and many of its members were dead. The following year, Jacques de Molay was burned at the stake after declaring that the Templars' confessions were lies obtained under torture. Stories would spread that as he died, he condemned Clement and Philip to join him within a year. Although it is impossible to say whether he truly called divine vengeance down upon them, within a few months' time both pope and king had gone to their graves.

The author's main point is that the Knights Templar:

A. grew to possess power far beyond what might be expected from their humble beginnings.
B. were actually a more powerful force in Europe than they were in the Holy Land.
C. possessed great power as an organization, which ironically brought them to ruin.
D. possessed sufficient power to challenge the rule of monarchs.

QBanks: Refunds are considered for subscriptions of more than 30 days. Refund amounts are based on the number of days elapsed since the subscription start date or the number of questions used, whichever amounts to a higher deductible.

Example scenario: An individual purchases a 180-day Qbank subscription and generates a few test blocks equal to 20% of the questions in the QBank. After 45 days, he/she requests a refund. Because the elapsed duration is greater than the percentage of questions used, the refund amount will be based on a subscription converted to the closest duration (60 days) by deducting the applicable 60-day subscription fee from the refund. In the same scenario, if the individual had used 85% of the questions, the refund would be 15% of the subscription fee paid.

Note 1: A processing fee of 10% (minimum of $10 and maximum of $25) will be applied to all refund amounts.

Note 2: All refunds, pro-rated or otherwise, must be requested within 360 days of original purchase. Refunds for purchases made more than 360 days ago will be issued as non-refundable, non-transferable store credit.

Note 3: Subscribers must notify UWorld of their intent to obtain a refund before activating their subscription (or) during the active duration. If the reset option is used or all of the questions are used, no refund will be allowed (see “I want to reset/delete my QBank test history (or) start all over again, is this possible?” below for further details). No refunds will be offered for expired subscriptions.

Note 4: For combination package purchases, individual subscription fees will be applied when calculating the refund. If any self-assessment Exam(s) is utilized from a package, the entire purchase is rendered non-refundable.

Note 5: Although access to QBanks via certain mobile devices is provided as an additional feature, it is not intended to be a replacement for computer access, and no refunds or cancellations will be honored due to user inability to install and/or use the provided software on any mobile device (regardless of whether the device meets the minimum system requirements).

What is your refund policy?

All direct UWorld purchase refund requests are evaluated case-by-case. The refund amounts determined are final.

Course Purchases

Courses purchased within 1-week of activating or receiving printed material, whichever is earlier, can receive a refund. The following conditions apply:

  • No more than 10% of the course (including but not limited to Video Lectures, QBank, Flashcard Quizzes, UBooks, or a combination of any) may have been used
  • A cancellation fee of 10% (minimum $10) will apply
  • Any shipping charges will be deducted from the refund
  • Return all printed material in new condition

If your course includes printed materials, the refund will be processed only after we receive the materials and verify their condition. Please return the printed materials to the address below.

AAMC Exam

Redeemed: A $140 activation fee applies if any AAMC Exam is redeemed. Access to all purchased exams is retained through the AAMC website.

Unredeemed: If no exams are redeemed, all exams are revoked. No activation fee is applied.

AAMC Prep Hub Resources

Clicking on “Go to AAMC” within the AAMC tab of the Comprehensive Prep Course activates your AAMC subscription.
